Plan of Care and Hourly Rate Life Changer is a Residential Services Agency (RSA) located in Hyattsville, Maryland. We offer affordable in-home care services 24 hours a day, 365 days a year. Our care coordinator will work with you to set a care plan and schedule that works best for you and your family. Our plan of care starts with a free Registered Nurse (RN) assessment, and we assess our client’s health condition every three months.

Our services include:
· Personal care
· Nursing care
· Homemaker services

· Bathing/Dressing, Meal Preparation, Medication Reminders, Cleaning, running errands, Companionship, Monitoring, Mobility, Transferring, Incontinence Care etc.

Hourly Rate/Charge
Our hourly Rate depends on the services we offer. We have Home Health Aids (HHA), Certified Nursing Assistants (CNA), Licensed Practical Nurse (LPN) and Registered Nurses (RN). We do background checks for all our employees.

a. Home Health Aid (HHA): $22.00-$25.00
b. Certified Nursing Assistant (CNA): $25.00-$28.00
c. Licensed Practical Nurse (LPN): 30.00-$ 35.00. Our Registered Nurses (RNs) do quarterly assessment of our clients (free).
